Abstract
Total contact cast（TCC）は長年にわたり米国における糖尿病足病変のゴールドスタンダードである．TCC は膝下から足部に密着するように作成されたキャスト（ギプス）であり，体重を下腿と足底全面（total）に密着させ（contact）分散することで，糖尿病性知覚障害によって知覚のない足部を歩行中の度重なる足底圧の集中やズレから守り潰瘍を治癒へと導く．本稿においてはその効果と手技，合併症などについて述べた．TCC の作成においては手技の習熟が必要で合併症に十分注意が必要ではあるが，足底潰瘍に対する治療効果については複数のRCT でエビデンスが得られている．今後本邦における多施設研究においてもその効果と合併症を検討し，詳細を報告する予定である．
