The increasing frequency of methicillin-resistant Staphylococcus aureus (MRSA) infections has led to renewed interest in clindamycin therapy for such infections.1 Some MRSA strains that are clindamycin-susceptible and erythromycin-resistant have inducible clindamycin resistance. There are 2 genes that cause erythromycin resistance in MRSA strains; one confers resistance to clindamycin and one does not. Differentiating between these 2 types of erythromycin resistance can be accomplished with a double disc diffusion assay or D-test. Investigators from Johns Hopkins University, Baltimore, Md, report a 5-year-old girl who failed clindamycin treatment for a clindamycin-sensitive, erythromycin-resistant MRSA surgical infection after craniosynostosis surgery.
