Abstract

Despite limited sensitivity of Tl-201 brain SPECT in detecting infratentorial small neoplasms, a case of glomus jugulare tumor or nonchromaffin paraganglioma visualized on a Tl-201 tomographic study is reported. A 68-year-old man who underwent surgical resection of prostate cancer 2 years earlier was examined for a 6-month history of recurrent headaches, left-sided pulsatile tinnitus, and hearing loss. Magnetic resonance imaging (MRI) of the brain showed a small hypointense mass with enhancement after gadolinium-DTPA administration in the posterior cranial fossa at the level of the left jugular foramen. A possible glomus tumor was considered. Tl-201 brain SPECT showed a small focus of abnormal tracer accumulation in the left temporal area, adjacent to the cerebellopontine angle, with a lesion:contralateral normal region uptake ratio greater than 3. A digital angiographic scan revealed a glomus jugulare tumor in the left jugular foramen, which was confirmed by histopathologic examination after excision.
