Abstract

A 43-year-old female had sudden onset of shortness of breath associated with sharp generalized chest pain, radiating to the back. On the arrival to the emergency room, patient was tachycardic and tachypneic with a third heart sound (S3) on exam. EKG showed sinus tachycardia with ST elevation in V2–V4, T wave inversion in I, aVL, V3–V6, and diminished R waves in V1–V3. Cardiac enzymes were elevated with a troponin of 24 ng/ml (normal: 0.00–0.09) and CK-MB of 121 (normal: 0–5 ng/ml).
