Abstract
In the treatment of intestinal obstruction of the left-sided colorectal cancer, transnasal ileus tube can not provide enough decompression in a short time and often leads to emergency operation. In addition, we often have difficulty in performing an operation on an one-step approach and that resulting in a radical operation. In our institution we employ the following procedures for intestinal obstruction of the left-sided colorectal cancers; first, the obstructed site and its condition are confirmed by barium enema study using gastrographin; second, a guide wire is inserted into the oral side of the obstructed site under a fiberoptic colonoscopy and the colonoscope is pulled out; and finally, under fluoroscopy, a transanal ileus tube with a devised tip is inserted into the lumen guided by the guide wire. Immediately after insertion of the decompression tube, the intestinal obstruction is relieved that enables us to conduct satisfactory preoperative examinations and pretreatment of the large intestine. This paper presents these procedures and the availability in three such cases in which an operation on an one-step approach could be safely performed.
