Abstract

A 78-year-old man with a history of Paget's disease and right hip pain was evaluated by whole body Tc-99m MDP bone scintigraphy. The activity of Paget's disease can be assessed by bone scanning. Quiescent portions may appear normal while active areas of osteolysis or osteoblastosis show increased uptake, ft has been shown previously that the peripheral margins of osteoporosis circumscripta demonstrate increased uptake relative to surrounding bone or even to its central portion. We describe the bone scan findings in a case of mixed osteolytic and osteosclerotic Paget's disease of the skull, resulting in the appearance of a “yarmulke” sign.
