Abstract

Twenty-two medially based fasciocutaneous flaps in 11 cases with chronic burn contractures in the popliteal region are presented. In 5 patients, 2 adjacent pedicled flaps were used to cover the defects after removal of the contractures. In another 6 cases, 2 separated pedicle flaps were utilized to cover the secondary defects created after proximal and distal releasing incisions. Donor areas were closed primarily when possible, this was done in 7, skin grafting was used in the other 15 flap donor areas. Only 1 of the 22 flaps, this being in the first group, was observed to have partial necrosis, the remaining flaps healed uneventfully with complete survival. The follow-up period was 18–24 months. This new approach to long-standing burn contractures of the popliteal region has the advantages of simplicity, short hospitalization, a better functional result and avoidance of recurrence, in selected cases.
