The treatment strategies of adult degenerative scoliosis

Abstract

Objective To discuss the progress of adult degenerative scoliosis(ADS) treatment strategies. Methods The researches related to the treatment of ADS were searched in, Medline, EMbase, CNKI, VIP database and Wanfang database from January 2005 to December 2014, by usingscoliosis, degenerative, adult, treatment strategiesand脊柱侧凸,退变性,成人,治疗策略as key words. Results ADS has been made a slow progress, the incidence gradually increased with age. Asymmetry disc degeneration and facet joint degeneration is the root cause of the ADS. It may take a different clinical treatment based on clinical symptoms and radiological examination of the patient. For mild symptoms, no significant back pain and radicular symptoms, radiographic spinal stenosis is not obvious, Cobb angle <30°, the side slip<2 mm with front osteophytes, coronal and sagittal balance relatively reasonable person. It can be treated conservatively. For severe symptoms, conservative treatment fails, the patient deformity progresses, the need for surgery. Surgical methods include simple spinal decompression, fusion and orthopedic osteotomy. It should be based on the patient's condition, choose one or a combination of several ways to surgery. Conclusions ADS, mainly in the elderly population, is a complex pathological processes. Fully understanding and grasping the characteristics and laws of the disease, the patient system, a detailed condition assessment, development of scientific, standardized and effective individualized treatment, can achieve better clinical efficacy.
