Abstract

Although a number of outstanding techniques have been developed for the stabilization of intra- and juxta-articular fractures of the hand, one may encounter difficulties with conventional methods of treatment in some large osteochondral and intra-articular shear fractures of the metacarpus and the proximal phalynx. Absorbable implants have been used successfully in the management of such fractures. Secure stabilization was achieved with the use of biodegradable rods of an appropriate diameter which were placed through the articular surface using a predrilled tract. We have used biodegradable implants in 46 patients without any unfavorable reactions to the polymer. Failure of union was observed in four patients and was related to technical problems unrelated to the device. Absorbable implants used in the manner described enable satisfactory stabilization of difficult small fragment fractures without interference of joint movement and function.
