INNOVATION AND IMPROVEMENT: St Paul’s Hospital implemented a CR in two community centers in Vancouver and was ready to assist a rural community. The two main communities of Gibsons and Sechelt had new recreation centres and equipment necessary for a CR. A partnership with Vancouver Coastal Health existed. The local internist became the intake point, a nurse took on the role of case manager and the Recreation Centre provided an Exercise Specialist. June 2012; 4 people start in Sechelt, group full at 10 by September. September 2012; group starts in Gibsons and full 6 weeks later. January 2013; Maintenance groups are started for fresh graduates from 6 month program. Wait list is growing. September 2013; group size increased from 10 to 12. January 2014; program length changed to 4 months in response to long wait list. CONCLUSION: No adverse events have taken place and patient satisfaction has been high. Community partnerships can lead to successful CR programs in rural areas.
