Abstract
目的 探讨股骨颈疝窝的影像学表现和发病机制.方法搜集18例经病理证实或具有典型影像学征象的股骨颈疝窝影像学资料.全部病例均行髋关节X线平片检查,16例行CT检查,13例行MR检查,8例行放射性核素显像[发射型计算断层摄影(emission computed tomography,ECT)]检查.结果 18例共22个病灶,均位于股骨头基底和股骨颈前外侧皮质下,最大径线通常＜10 mm,可呈圆形或卵圆形.X线平片上表现为伴清晰薄层硬化缘的透亮区,CT上表现为伴锐利的薄层硬化缘的骨质缺损区,并在病灶或上下相邻层面上前方皮质显示与病灶相通的局部裂隙样缺损.MRI上病灶可呈3种异常信号.ECT上大多数无明显异常.结论股骨颈疝窝位于股骨头基底和股骨颈近段前部皮质下,多伴有清晰锐利的薄层硬化缘.局部皮质与病灶相连的裂隙样缺损,是诊断股骨颈疝窝较为特异的征象。
