Abstract
The results of experimental and clinical studies indicate that in patients with diabetes, the stomach is often the cause of various dyspeptic symptoms, and the impaired gastric emptying may be accompanied by a worsening of glycemic control. The use of drugs that acts as endogenous incretins, inhibits gastric emptying and improves glycemic control. Different methods of bariatric surgery in patients with obesity and type 2 diabetes reduce food intake and body weight, and their effect on glycemic control precedes weight loss and does not correlate with the degree of weight loss. Currently, the mechanisms responsible for improving glycemic control after bariatric surgery are intensively studied, such as increasing the level of circulating incretins stimulated by the release of nutrients in the intestine and contributing to weight loss regardless of glycemic control. It was concluded that the stomach should be considered not only as a negative, but also as a positive modifying factor, the effect of which can improve the results of treatment and prolonged management of patients with type 2 diabetes

У больных сахарным диабетом (СД) при его длительном течении желудок обычно является источником появления как хронических диспепсических симптомов, так и нарушенной толерантности к глюкозе. И, наоборот, модулирование функции желудка фармакологическим или хирургическим путем приводит к улучшению гликемического контроля у больных сахарным диабетом 2 типа. После приема пищи имеет место интегрированный гастроинтестинальный ответ, который включает в себя моторные, секреторные и пищеварительные процессы, а также сопутствующее выделение глюкозы под влиянием регуляторных гормонов, таких как инсулин, глюкозозависимый инсулинотропный пептид и глюкагоноподобный пептид-1 (ГПП-1) [3].
