Abstract

Background: The Texas Diabetes Prevention and Control Program at the Texas Department of State Health Services contracts with community-based organizations, including federally qualified health centers, local health departments, and other non-profits. Fifteen Community Diabetes Projects (CDPs) increase opportunities in the state for implementing positive behavior and lifestyle changes in people with diabetes and those at risk by targeting Texans who have disproportionate rates of diabetes, and limited access to health services. Interventions include diabetes self-management education (DSME), nutrition classes, and physical activity.
