Abstract

ABSTRACTObjective: Trendelenburg's theory was based on theconcept of a descending valvular incompetence com­mencing at the saphenofemoral junction (SFJ) withsubsequent distal progression ofreflux. The aim ofthisstudy was to evaluate the distribution of reflux inpatients with superficial venous incompetence in order toassess validity ofthe above hypothesis.Procedures: The superficial and deep venous system ofaconsecutive series of 443 patients presenting withprimary varicose veins was examined by one vasculartechnologist using colour flow ultrasonography.Results: Six hundred and eleven lower limbs in 443patients were examined. The distribution of longsaphenous vein (LSV) reflux was the following: Ofthe611 limbs 454 showed LSV reflux, with 240 legsexhibiting total reflux (SFJ and LSV) and 214 legsexhibiting reflux in evolution (LSV reflux withcompetent saphenofemoral valve, isolated posteriorarch reflux and lateral anterior thigh vein reflux withSFJ reflux).Conclusion: These results suggest that reflux startsdistally and progresses proximally, thus throwing theTrendelenburg theory into dispute.Keywords: Saphenofemoral valve; Long saphenousreflux
