Abstract

The PTFE grafts have been used as vascular access for hemodialysis patients for a long period of time. The most common complication of the grafts was thrombosis. The treatment of thrombosis has consisted of surgical revision of the stenosis or balloon thrombectomy. An alternative treatment is via endoscope and grasper for thrombectomy with only one incision. From June 1996 to January 1998, 16 patients received endoscopic thrombectomy. The cumulative patency rate at 3 months was 87.5%, 75% at 6 months, 68.7% at 9 months and 62.5% at 12 months. Rupture of the vessel wall at the Gore-Tex venous anastomosis site occurred in one patient. Patch grafts of the venous end due to hyperplasia of the venous intima were done in 2 grafts. Another 2 complications of immediate obstruction also occurred. The initial result was acceptable. Further study of the long-term result was necessary.
