Abstract
This chapter explores the systems within which we work and some of those that are external to our core organizations. Both formal and informal systems are explored. The chapter focuses on information flow (how information moves about), information sets (what is stored), and information systems (where and how it is stored). Systems problems are a common source of error. The tendency to blame an individual rather than seek the system cause is discussed. Open versus leaning versus blame cultures are considered.
