Abstract

Systemically administered isotretinoin has proved to be the single most significant advance in acne therapy. Isotretinoin produces complete or near complete clearing of acne in nearly all patients, with prolonged remission and “cure” occurring in many.1-3 This dramatic improvement is the result of isotretinoin counteracting all areas of acne pathophysiologic evidence (ie, excessive sebum production, abnormal desquamation of follicular epithelial cells (comedogenesis), proliferation of Propionibacterium acnes in the environment of excessive sebum and abnormally shed follicular cells, and inflammation generated by proinflammatory substances produced by P.
