Abstract

Various benign and malignant conditions could cause extraluminal biliary obstruction. Compression of extrahepatic bile duct by the anteriorly overriding right hepatic artery was reported as a right hepatic artery syndrome. On the best of our knowledge, only 7 cases of this entity were reported up to date. The authors present a case of the 54-yr male patient with clinical, laboratory, US and CT signs of hilar biliary obstruction, mimicking Klatskin tumor. During the surgery, right hepatic artery syndrome was confirmed by intraoperative cholangiography. Problem was solved by the careful dissection of the right hepatic artery and deliberation of the common bile duct. The patient remains symptom-free with normal clinical and laboratory data more than 5 years months after surgery.
