Abstract

Objective To study the model of comprehensive prevention and control on hypertension and diabetes in community residents.Methods A community in Guangdong Province from January 2009 to December 2012 was randomly selected by random cluster sampling method.Residents in the community were followed up and investigated by the method of prospective health intervention.The status of community residents and patients with high blood pressure,diabetes was followed up before and after intervention.Results The survey participants included 5293 community residents in Guangdong province,and 4967 cases were followed up,with the follow-up rate of 93.84%.Prevalence rate of community residents was similar before and after intervention (24.32% vs.22.59%,P > 0.05).After intervention awareness rate of knowledge about hypertension,diabetes in community residents were significantly higher than those before intervention,with statistically significant differences (P < 0.001).Proportion of health behavior such as regular blood pressure measurement,regular blood sugar measurement,low salt diet and regular exercise after intervention in community residents were obviously higher than those before intervention,with statistically significant differences (P < 0.001); and the proportion of bad behavior such as smoking,alcohol consumption and highfat diet were significantly lower than those before intervention,with statistically significant differences (P < 0.001).After intervention,rates of treatment,rational drugs use and disease control were significantly better than those before intervention,with statistically significant differences (P < 0.001); rates of complications,disability and mortality in patients with high blood pressure,diabetes after intervention were significantly lower than those before intervention,with statistically significant differences (P < 0.001).Conclusion Chronic diseases' prevention and control depends on working together of government,society,residents and families,and community health intervention model is an effective way of prevention and control of chronic diseases.
