Abstract
Aim. To determine the rational volume of conservative therapy with form of hypotonic biliary dyskinesia and vegetative disorders in patients with chronic acalculous cholecystitis (CAC). Material and methods. The comparative retro- and prospective analysis of conservative treatment in 114 patients with CAC, who had been treated in the clinic the clinic of surgery. K.T. Ovnatanyana DonNMU of M. Gorky from 2008 to 2015. Among them were 67 females (59.1%) and 47 (40.9%) males at the age of 43,0 ± 6,7 years. The following methods were used to patients: biochemical blood analyses, ultrasound of hepatopancreatoduodenal area, fractional chromatic duodenal intubation with microscopic, biochemical and bacteriological studies of bile, the dynamic ultrasound examination of the gallbladder with test meal. All patients were divided into two homogeneous groups of 70 people. 1st group (main one) received treatment based on the type of dyskinesia of gallbladder and vegetative disorders (Patent of the Russian Federation № 2589900 from 02.06.2015), and 44 people. 2nd group (compare one) got only standard conservative therapy (table №5, holekinetiki, choleretic and others.). Results. The treatment program of CAC consisted of dietotherapy, drug therapy during the exacerbation; pain relief; using of choleretic drugs; antibiotic therapy; normalization of the functions of the autonomic nervous system; immunomodulatory therapy, and improving of the general reactivity of the organism. At the hypotonia gallbladder good effect was obtained of using of Gerbastress with Polyphytohol and Vitam (Patent of the Russian Federation № 2589900 from 02.06.2015). In the 1st group various complications were noted in 2.1%, in the 2nd group - in 5.4%. In a remote period (after 1-5 years) the best indicators of the quality of life were in the 1st group. Conclusions. Using developed an improved method of treatment causes a more expressed in comparison with the control group, a reduction of pain, dyspeptic and asthenic-vegetative syndromes, improve the functional state of the biliary tract and biochemical indices of bile, improving the quality of life on all scales by 1.3-1.7 times .
