Abstract

SummaryIn an analysis of a series of 12 636 consecutive deliveries at the Riyadh Armed Forces Hospital, 1103 (8.7 per cent) were undergoing their eighth or subsequent pregnancy. The maternal and perinatal mortality rates rose with increasing parity. There was an increase in the incidence of breech presentation, cord prolapse, the need for obstetric intervention by caesarean section, multiple pregnancy and post-partum haemorrhage in the grand multiparous patients. Haernorrhage during pregnancy related to premature separation of the placenta and bleeding of undetermined origin were most common in grand multiparous patients. The forceps and ventouse delivery rate was higher in primiparous women and least frequent in the grand multiparae. There was one maternal death but no cases of ruptured uterus. Grand multiparity is still an obstetric problem.
