Abstract

Objective To study the changes of serum tumor necrosis factor (TNF)-a, β2- microglobulin (MG), intercellular adhesion molecule (ICAM) in patients with systemic lupus erythematosus (SLE) with renal histopathological changes, and then discuss their mechanism of action on early renal damage of lupus nephritis (LN). Methods 67 cases of SLE patients underwent renal biopsy guided by B ultrasound for pathological type, and were divided into type I group (24 mild mesangial cases), type II group (22 mesangial proliferative cases), type III group (21 cases of focal active or inactive lesions) according to the pathological type. 25 cases of healthy controls in our hospital were set as healthy control group. Serum TNF-a and ICAM levels were detected by enzyme linked immunosorbent assay (ELISA), and serum β2-MG level was detected by radioimmunoassay. Analyzed the clinical manifestations, laboratory indexes of kidney. Results Serum TNF-a and ICAM levels were (4.46±0.46) ng/ml and (110.73±23.65) ng/ml in type I group, (11.48±2.07) ng/ml and (135.84±29.24) ng/ml in type II group, (25.69±2.43) ng/ml and (192.57±35.67) ng/ml in type III group, which were all higher than those in healthy control group [(1.76±0.53) ng/ml, (78.34±19.37) ng/ml], with statistically significant differences (P<0.05). Serum β2-MG level was (1 956.1±46.26)μg/L in type I group, (2 013.1±57.29)μg/L in type II group, (2 131.7±67.36)μg/L in type III group, which were all higher than that in healthy control group [(1 802.2±67.25)μg/L], with statistically significant differences (P<0.05). Serum TNF-a, β2-MG, and ICAM levels showed an increasing trend with the change of pathological types (r were 0.656, 0.715, 0.681; P<0.05). Conclusions The changes of serum TNF-a, β2-MG, and ICAM levels in patients with SLE are positively correlated with pathological type progress, which can reflect the severity of renal damage in patients with SLE to some extent. The detection of two indexes will not cause pain, convenient and feasible, especially suitable for those who cannot undergo kidney pathology. The clinical dynamic observation on serum TNF-a, β2-MG, and ICAM levels contribute to early diagnosing renal damage in patients with SLE, so as to provide experimental basis for early diagnosis of LN.
