Abstract

The ExPRESS glaucoma implant is a miniature stainless steel device that offers a simple and safe alternative to the classic trabeculectomy. It is a minimally penetrating glaucoma surgery (MPGS), with proposed advantages of standardizing filtration surgery with reduced risk of early postoperative complications and a short learning curve. However,the device is associated with unique intraoperative and postoperative problems in addition to a high cost which merits a serious review of its role in current glaucoma practice despite tremendous market pressures. We put forth this review for a balanced perspective on the merits, pitfalls and cautions on the use of this device, giving the implant details, indications, current surgical procedure for implantation, and a literature review comparing ExPRESS to trabeculectomy.
