Abstract
Feedback was obtained by email, phone and direct conversations with members of the NORCLEFT Surgical and Nursing SIGs until a consensus opinion was reached. Results/conclusion:The agreed proformas are being used for ongoing data collection in the 4 services, with initial audit results to be presented at NORCLEFT 2013. They may be used as the foundation for a database. The final proformas will be available at presentation.
