The effect research of implementing health management model on the patient with stroke in the community

Abstract

Objective To explore the rehabilitation effect of health management on the patient with stroke in the community.Methods Sixty patients with stroke who discharged from hospital were divided into study group (30 patients) and control group (30 patients) by coin dividing group method,the control group was given the self-management model,the study group was given the health management intervention that the medical staff in the hospital carry out,the observing time lasted 6 months.Before and after intervention,the effects in the two group was evaluated by HPLP Ⅱ,NFDS,ADL,and SF-36.Results Before intervention,the scores of HPLP Ⅱ,NFDS,ADL,and SF-36 in the two groups were not different (P ＞ 0.05),After intervention,the scores of HPLP Ⅱ,NFDS,ADL,and SF-36 in the study group were better than those in the control group (P ＜ 0.01).Conclusions Health management model could improve health management of the patient with stroke in the community,reduce illness deformity,and improve their life quality.
