Abstract

Introduction: Mortality and morbidity have been reduced with high-resolution typing and matching of major-immunogenic HLA alleles. Despite this, risks of transplant-related complications and relapse remain undesirably high. Evidence supports that additional matching of HLA-DPB1 promotes event-free survival. However, with an 80% mismatch rate it is not feasible to only select DPB1-matched unrelated donors. Revelations suggest permissive mismatching and avoidance of non-permissive mismatching can achieve similar benefits to DPB1 matching.
