(Anesth Analg. 2018;126:552–558) Spinal-induced hypotension, if not treated, can result in adverse maternal and neonatal outcomes (maternal cardiovascular collapse and fetal acidosis). Glycopyrrolate, an anticholinergic with quaternary amine structure, has limited ability to cross-membranes and may thereby have potential to reduce hemodynamic changes after spinal anesthesia in obstetric patients. The authors of this meta-analysis evaluated the literature to determine if prophylactic glycopyrrolate reduces the incidence of hypotension and total vasopressor dose required during elective cesarean delivery under spinal anesthesia.
