Abstract
Background Little Clinical attention paid to fatigue in patients with chronic obstructive pulmonary disease (COPD), and there are few relevant studies on the risk prediction of fatigue in acute exacerbation of COPD (AECOPD), which need to be further confirmed. Objective To explore the relationship between fatigue and AECOPD. Methods COPD patients were selected from three general hospitals in China to establish a cohort study population, between March 2019 to October 2020,and followed up to 12 months. A total of 550 cases were included in the study. Patients were investigated by a demographic questionnaire,Fatigue Scale-14,BODE index,Hospital Anxiety and Depression Scale and COPD Assessment Test during the hospitalization and follow-up. ROC curve was used to analyze the selected risk factors. Results Fatigue occurred in 416 COPD patients. There were significant differences in smoking, times of acute exacerbation, CAT score, BODE index between the two groups (P<0.05). Cox proportional hazards regression model analysis showed that acute exacerbation, CAT score, BODE index, were the risk factors of fatigue (P<0.05). The incidence of fatigue in patients with AECOPD≥2 times in the past year was 2.3 times higher than that in patients<2 times (P<0.01). ROC curve analysis showed that the area under the curve of fatigue anxiety in predicting AECOPD patients were 0.826 respectively, the sensitivity were 85.2% respectively, the specificity were 65.4% respectively, and the Jordan index were 0.506 respectively. Conclusion Fatigue increase the risk of AECOPD patients to a certain extent,Clinical targeted preventive interventions should be carried out to reduce the occurrence of AECOPD.
