Abstract

<b>Background</b> Little Clinical attention paid to fatigue in patients with chronic obstructive pulmonary disease (COPD), and there are few relevant studies on the risk prediction of fatigue in acute exacerbation of&nbsp;COPD (AECOPD), which need to be further confirmed. <b>Objective</b>&nbsp;To explore the relationship between fatigue and AECOPD. <b>Methods</b>&nbsp;COPD patients were selected from three general hospitals in&nbsp;China to establish a cohort study population, between March 2019 to October 2020,and followed up to 12 months. A total of 550 cases were included in the study. Patients were investigated by a demographic questionnaire,Fatigue Scale-14,BODE index,Hospital Anxiety and Depression Scale&nbsp;and COPD Assessment Test during the hospitalization and follow-up. ROC curve was used to analyze the selected risk factors. <b>&nbsp;Results</b> Fatigue occurred in 416 COPD patients. There were significant differences in smoking, times of acute exacerbation, CAT score, BODE index between the two groups (P&lt;0.05). Cox proportional hazards regression model analysis showed that acute exacerbation, CAT score, BODE index, were the risk factors of fatigue (P&lt;0.05). The incidence of fatigue in patients with AECOPD≥2 times in the past year was 2.3 times higher than that in patients&lt;2 times (P&lt;0.01). ROC curve analysis showed that the area under the curve of fatigue anxiety in predicting AECOPD patients were 0.826&nbsp;respectively, the sensitivity were 85.2%&nbsp;respectively, the specificity were 65.4%&nbsp;respectively, and the Jordan index were 0.506&nbsp;respectively. <b>Conclusion</b>&nbsp;Fatigue increase the risk of AECOPD patients to a certain extent,Clinical targeted preventive interventions&nbsp;should&nbsp;be carried out to reduce the occurrence of AECOPD.
