Abstract

The Orthopaedic Trauma Association (OTA) recently proposed a new classification of open fractures. 1 Orthopaedic Trauma Association: Open Fracture Study Group A new classification scheme for open fractures. J Orthop Trauma. 2010; 24: 457-465 Crossref PubMed Scopus (76) Google Scholar This pathoanatomic classification was produced by the OTA Classification Committee chaired by Dr. Larry Marsh and is based on five essential categories: skin injury, muscle injury, arterial injury, contamination, and bone loss. Many have wondered whether they should immediately stop using the Gustilo classification and start using this new classification. Others have rightfully asked: Is it reliable? Is it valid? Is this classification fully developed? How do you use it? We will try to address these questions after placing the new classification system in its historical context. First, we would like to declare a conflict of interest for one of the authors: the first author (CSR) now chairs the OTA Classification Committee, which developed this classification.
