Abstract

A 23-year-old White Australian male presented with hemoptysis 9 days after kidney transplantation. This was his second kidney transplant for Alport syndrome, with genetic confirmation of COL4A5 gene mutation. His first transplant, from his father, lasted 2 years and then failed due to a vascular rejection in the setting of nonadherence. His second transplant was human leukocyte antigen–DR matched and from a deceased donor with donation after brain death. The patient received standard induction with tacrolimus, mycophenolate mofetil, prednisolone, and basiliximab.
