Abstract

The treatment of iliac artery aneurysms is challenging. Conventional surgery is associated with high morbidity and mortality, which led to the development of less invasive alternatives, such as endovascular techniques. When performing an endovascular treatment of a common iliac artery (CIA) aneurysm, is essential to ensure good landing zones, which may require extending the distal landing zone to the external iliac artery (EIA), usually smaller than the CIA. To bridge this diameter difference between the proximal and distal landing zones, the off-label technique of reversal of a flared iliac stent graft was described. The authors describe a case of successful endovascular treatment of a CIA aneurysm, applying this technique.
