Abstract
Tail gut cystは仙骨前面に発生するまれな疾患で，悪性所見を伴うことはさらにまれである．今回，悪性化を伴ったtail gut cystの1例を経験したので報告する．症例は50歳代，女性．6年前に仙骨前面の嚢胞性病変を指摘され経過観察となっていたが，嚢胞の急速な増大とともに肛門部痛が出現した．骨盤内嚢胞から直腸への浸潤を伴う腫瘍を指摘され手術となる．手術は開腹直腸切断術・嚢胞摘出術を施行．外括約筋から大殿筋まで腫瘍が進展しており合併切除術を施行した．病理診断でtail gut cystから発生した腺癌と診断され，術後直腸癌に準じた化学療法を施行した．局所再発に対して追加切除を行うも局所再発が原因で術後3年目に永眠となった．悪性所見を伴うtail gut cystは完全切除が可能であれば予後は比較的良好であるが，癌遺残例では早期再発例が散見され，外科的根治切除が重要である．
