Abstract
The literature on aphasia with subcortical lesions is reviewed. Lesions of thalamic relay nuclei linked to the cortical language areas are related to paraphasic aphasia. The data with white matter lesions is less consistent. A number of reports describe propositional aphasia with lesions of the frontostriatal pathways, transcortical motor aphasia with lesions in the area of the knee of the internal capsule and paraphasic aphasia with lesions of the anterior limb of the internal capsule. Only few studies have found aphasia with lesions of the basal ganglia proper. In general, subcortical aphasias are transient. Models of the participation of the subcortical nuclei in language functions have to consider recent developments in our understanding of their organization and function.
