Abstract

Stakeholders from thirteen integrated care initiatives across Europe collaborated with SUSTAIN project partners to design and implement improvement plans tailored to local priorities. Identified improvement areas in the integrated care initiatives included: (i) increasing engagement of older people and informal caregivers in decision-making and planning their care process, and (ii) improving collaboration and information exchange between professionals and organizations involved. Despite differences in the contexts and characteristics of the integrated care initiatives, similar facilitators and barriers towards improving current practices were identified, including motivated staff and lack of financial and personnel resources. During this session, we will present the results of an overarching analysis in which we compared experiences and outcomes related to the design and implementation of the improvement plans in the different initiatives. As such, we aimed to obtain insight into what works for whom and in what context and to generate evidence on further developing integrated care.
