Abstract

Due to the increased life expectancy and exposure to high-energy traumas, there has been a rise in the demand for surgical treatment using internal fixation, and consequently, an increase in refractures. Despite the growing discussion around peri-implant fractures, there is still little discussion addressing periplate fractures. This report presents a case using the plate-on-plate technique where the fracture is addressed while maintaining the previous fixation material by placing a locked plate above the previously existing material. This case involves a 54-year-old man with a peri-implant diaphyseal fracture of the distal third of the left femur after a motorcycle versus car collision. The patient underwent surgical treatment with a steel locking condylar plate, maintaining the previous fixation material through the plate-on-plate technique. Subsequently, fracture consolidation and functional range of motion were evidenced. Thus, in line with studies, the plate-on-plate technique proved to be an effective alternative for peri-implant fractures, allowing an approach with lower morbidity and mortality and reducing the risks of surgical complications; however, it requires studies with a higher level of epidemiological evidence.
