Abstract

Objective To investigate the clinical features and to evaluate the effects of different surgical procedures of V pattern strabismus. Methods 67 cases of V pattern strabismus were retrospective studied,including 46 cases ofV exotropia and 21 cases ofV esotropia. The inferior oblique muscle weakening procedure or vertical transposition of horizontal rectus muscles were performed in patients with V pattern strabismus according to the function of inferior oblique muscle and with or without extortion. All patients were corrected horizontal deviation according to the angles of strabismus in primary position. Eye position, the function of oblique muscle and binocular vision were analyzed before and after surgery. Besults In 49 cases performed recession or transposition of the inferior oblique muscle, 43 cases acquired satisfactory alignment of the eyes without V pattern after surgery. Vertical transposition of horizontal rectus muscles were performed in 13 patients with overaction of inferior oblique muscle + or without overaction of inferior oblique muscle, and 11 cases acquired alignment of the eyes without V pattern. 19 cases of 67 were recoveried binocular vision after surgery. Conclusions The inferior oblique muscle weakening procedure was most effective for V pattern strabismus with overaction of inferior oblique muscle ++~+++.The horizontal rectus vertical transposition was most effective for V pattern with overaction of inferior oblique muscle +or without overaction of inferior oblique muscle. Different surgical procedures should be selected according to hyperftmction degree of inferior oblique muscle.
