Abstract
An extensive mitral annular calcification presents a formidable technical challenge during mitral valve surgeries. A 68-year-old female presented to our hospital for surgical treatment of severe mitral stenosis with heavily calcified annulus. She had a background of chemotherapy and radiotherapy for Hodgkin's lymphoma 12 years earlier. In view of heavy and extensive calcification of mitral annulus and subvalvular apparatus, a mitral valve replacement (MVR) using standard prosthetic valve was not possible and she underwent surgical replacement with a transcatheter aortic valve- an Edwards Sapien XT valve. She was not suitable for percutaneous MVR because of heavily calcified anterior mitral leaflet. To our knowledge, this is the first published report of surgical MVR with Edwards Sapien valve.
