Abstract
Surgery-First-Orthognathic-Approach (SFOA), defined as orthognathic surgery without preoperative orthodontic treatment, is a new paradigm in the combined orthodontic-orthognathic approach to jaw deformities. Refinement in maxillary surgery, especially posterior impaction of the maxilla and rotation of the occlusal plane markedly diminished the necessity for extraction of upper premolars to get sufficient reverse overjet in mandibular set-back surgery for Class III patients. Patients with mild to moderate crowding and acceptable arch coordination can receive orthognathic surgery without preoperative orthodontic treatment. With considerable preoperative planning and precise surgery, the postoperative orthodontic procedure is direct and rapid; patients can get the desired facial profile and occlusion in much less period of time.
