Abstract

We read the paper entitled ‘A reappraisal of the ileo-rectal anastomosis in ulcerative colitis’ by Myrelid et al. with great interest.1 Ileo-rectal anastomosis [IRA] was performed frequently before the 1980s. After the 1980s, ileal pouch anal anastomosis [IPAA] replaced IRA as the first surgical choice in most cases. As the authors recommend, we choose IRA only in selected cases, particularly young females who are expecting pregnancy in the near future. The major concerns after performing IRA are the occurrence of inflammation and colitis-associated cancer [CAC] in the remnant rectum. It is correct that the risk of CAC after IRA …
