Abstract

The quality of healthcare services provided to patients can have a significant impact on their long-term health. It is unknown whether a patient’s perceptions of the quality of healthcare services they receive truly correlate with preventive health service delivery from their care providers. To determine whether an association exists between patients’ ratings of the quality of healthcare services that they have received and ever having a colorectal cancer screening test. The Health Information National Trends Survey (HINTS) 4 cycle 3 data was used for this study. Our analytic sample consisted of 1,562 respondents (weighted population size of 76,628,000) who were at least 50 years of age, had no history of colorectal cancer, and had rated the quality of healthcare services that they had received at their provider’s office in the previous 12 months. The respondents also stated whether they had ever received a colorectal cancer screening test in the past, including a stool blood test, sigmoidoscopy, or colonoscopy. We used survey weights in all analysis and logistic regression models were used to calculate odds ratios (OR) and 95% confidence intervals (CI). The mean age of the respondents included in the study was 64.9 years. The analytic sample comprised of 65% whites, 15% blacks, 14% Hispanics and 6% other races. The respondents consisted of 61% females and 95% had health insurance coverage. Seventy six percent (76%) of all respondents had undergone colorectal cancer screening in the past. Approximately 40% rated the quality of their healthcare as excellent, with 83% of this group of respondents having had colorectal cancer screening in the past. Comparatively, the groups who rated their care as very good, good, or fair/poor had a progressively smaller proportion of respondents who had ever undergone colorectal cancer screening at 74.4%, 68.1% and 59%, respectively. (Table). Patients who reported lower quality of their healthcare services were less likely to have undergone colon cancer screening tests.Tabled 1Ever screened for colorectal cancer by perceived healthcare qualityRatings of healthcare receivedEver screened for colorectal cancerWt % screenedUnivariateOR (95% CI)MultivariateOR (95% CI)Excellent(N=609)83.0ReferenceReferenceVery good(N=590)74.40.59 (0.36-0.98)0.61 (0.35-1.07)Good(N=275)68.10.44 (0.28-0.69)0.52 (0.31-0.89)Fair/Poor(N=88)59.00.29 (0.15-0.58)0.43 (0.16-1.17)Adjusted for age, gender, smoking status, highest education level, marital status, health insurance coverage, BMI, and race Open table in a new tab
