Water immersion “floats” the mucosa and submucosa away from the deeper muscularis propria, which may make EMR technically easier and safer. Thermal coagulation under water immersion may protect against injury to deeper wall layers (‘heat sink’ effect ). Colonoscopy using water immersion has generated growing interest as an alternative to air-insufflation with potential advantages of enhanced patient comfort, decreased sedation requirements and an improved adenoma detection rate. Conceivably, water immersion polypectomy and EMR could be performed seamlessly in sequence without any air insufflation.
