Abstract

This paper presents an operational research conducted to strengthen the service delivery of public health system in East Godavari District in Andhra Pradesh. This research was motivated by a situation where the poor were found opting for private hospitals despite prodding from government staff during a Dengue outbreak in July 2013 in a coastal village. Broadly, the research found that the underlying causes were the distorted incentive structure of doctors, organizational issues and gaps in supply chain of medicines. This paper presents a quantitative as well as qualitative analysis on the various factors which render the public hospitals unattractive for poor patients who are not rich enough to afford public hospitals. Further, it presents operational suggestions to make public hospitals more competitive and policy level changes for compensation system to doctors and organization of public health care to reorient the incentive structure of health personnel towards better service delivery at government hospitals.
