Abstract

AbstractThis article describes a project undertaken to enhance patient‐centred care in a cardiothoracic surgical unit in Melbourne. In particular, the article describes the implementation of a transdisciplinary patient assessment tool and interdisciplinary team meetings to enhance the understanding of the patient's experience of illness and to improve the coordination of care.
