Abstract

This report is the main deliverable of the Strategic Intelligence Monitor on Personal Health Systems, Phase 2 project (Simphs 2). It is based on an analysis of developments in integrated care in thirty-one care schemes across eight different Members States: Denmark, the UK, Spain, Italy, Estonia, the Netherlands, Germany and France. It summarises the main outcomes of the research, suggests areas where policy input is needed and proposes the development of a monitoring framework to enable decision makers to assess the state of maturity of the deployment efforts in a mainstreaming process.
