Abstract

Objective From the meaning of case management, to lay the foundation of stroke case management by analyzing the status of stroke patients continuity care in partly general hospital in china. Methods The nurses from 55 secondary and tertiary hospitals were surveyed by convenience sampling with homemade questionnaire. Results Of the 55 hospitals, only 7 (15.9%) case managers were responsible for the discharge plan, and 6 (13.3%) for follow-up after discharge. The main stagnation factors of stroke case management included lacking of information connection/sharing platform (81.8%) , lacking of case manager qualification mechanism (80.8%) , lacking of leading role among hospitals and communities (70.9%) , etc.. There was no significant difference in items between secondary and tertiary general hospitals in stroke continuity care (P>0.05) . Conclusions Continuity care of stroke patients in most of general hospitals does not have desired effects, and case management is a good choice as a mature continuity model for nursing management of stroke patients.
