interview, history, as well as Hamilton Depression Rating Scale scores <5 and Young Mania Rating Scale scores <5 on the day of the MRSI study. The average + SD years of illness was 17.5 + 9.4. All patients had agreed to discontinue any medication they were taking 1 week prior to their examination and were carefully monitored for re-emergence of symptoms. 3 ~p MRSI was performed using a spin-echo sequence (TR = 350 msec, TE = 3.5 msec) with three-dimensional gradient phase encoding and MRSI data volumes were processed on a micro-VAX workstation. ~JP spectra selected from a single voxel (25 cm 3) on T2 weighted MR axial images in each of the right and left frontal lobes were fit by NMR-I curve fitting software. No structural abnormalities, anatomical asymmetries, or atrophy of the frontal lobes were noted qualitatively on MR images of the patients and controls. Repeated measures ANOVA revealed that compared to the comparison group, the patients with bipolar disorder demonstrated significantly lower phosphomonoesters (PME) (p = 0.009) and increased phosphodiesters (PDE) (p = 0.025) in both the fight and left frontal lobes. The patient group also had significantly higher right compared to left frontal lobe phosphocreatine (PCr (p = 0.024). No other group differences or lateralized asymmetries were noted. The results provide support for altered frontal lobe phosphorous metabolism in bipolar disorder.
