Abstract
症例は64歳, 女性。右季肋部痛で発症し, 胆嚢結石による急性胆嚢炎の診断で保存的治療により一旦は軽快した。半年後再び症状が出現し, 肝右葉に嚢胞性病変を認め, 急性胆嚢炎の再発とこの間に発生したbiloma (胆汁性嚢胞) と診断した。症状改善とともに嚢胞は縮小し, その後胆嚢摘出術を行った。胆嚢頸部に径1cm大の結石が嵌頓しており, 胆嚢底部で肝床側の壁の一部が肝内に伸展, 穿破したと思われる炎症性瘢痕が見られた。総胆管結石症によるspontaneousbilomaの報告は多く, そのほとんどは術前に経皮的嚢胞ドレナージが行われている。急性胆嚢炎に由来する症例は稀だが, 保存的治療により胆嚢炎が軽快する場合, bilomaの自然縮小が見られ, 必ずしも経皮的嚢胞ドレナージは必要ではないと考えられた。
