Abstract

Massive gastrointestinal hemorrhage after erosion by prosthetic grafts has been reported as one of the most serious complications of aortic surgery. Clinically, however, it is difficult to differentiate bleeding from simple erosion by the graft from aortoenteric fistula. A case is presented in which bowel wall erosion, graft contamination, and aortic suture line disruption occurred in that order. Erosion is probably an important and early link in the chain of events leading to aortoenteric fistulization and in all such cases the contaminated graft must be removed. Several important preventive measures to be carried out during the initial procedure are mentioned.
