Abstract

Background Lateral window approach for sinus augmentation technique is a well-documented. Various grafted materials in sinus have been used such as xenografts, allografts, and it demonstrated good outcomes, but most of these grafts only with osteoconductive properties and had prolonged graft-healing time, therefore, the amount of new mineralized bone could be compromised. Recently reported that the sinus augmentation without bone graft might be considered effective impredictable bone formation. Aim/Hypothesis The aim of this reported was a case series of sinus augmented with or without bone graft materials over 5-year follow up. Material and Methods Totally 10 patients with mean age 55.4. Fifteen sinuses lift with simultaneously 40 dental implants placed in augmented sinus. Among the augmented sinuses 7 grafted and 8 without grafted bone materials, 3 sinuses used L-PRF without bone grafted in thickness Schneiderian membrane which portentously for sinusitis complication. The lateral window shift to inner site as tenting to avoid membrane drop down. For grafted site, filled with Bio-Oss to supported membrane, the non-grafted site used collagen membrane and implant supported the membrane. The dental implants length insertion from 10 mm to 13 mm in mean residual bone height 4.3 mm. The lateral window cover with resorbable membrane with fixation pin with primary closure the wound. Submerged healed 6 months, then uncover the implants. All implant restorations via provisional restoration observation occlusion comfortable and stable then definitive prostheses delivery. The observation evaluations through CBCT measurements. Results All of implants firm primary stability were achieved, giving implants healed above 6 months. Marked bone formation around implants observation with CBCT. One implant failed during provisional prosthesis stage, giving the survival rate of 97.5%. Conclusions and Clinical Implications The advantages of one stage implant placement without bone grafts in sinus lift procedure are easily to identify the new bone formation around the implant, avoid bone grafts related infection complication, and reduce costs effect. The used L-PRF in sinus lift procedure may enhancing osteoprogenitor cells and modulate hard tissue transformation. It is suggested that used non-grafted procedure in sinus augmentation may be an predicable alternative treatment options.
