Abstract

Median nerve compression above the elbow is rare. Entrapment of the median nerve at this level is due to a supracondylar process or a ligament arising from the supracondylar process extending to the medial humeral epicondyle. Clinical symptoms are similar to those of median nerve compression below the elbow, but pain localization is almost 5 cm above the elbow. At the same level it is possible to palpate a tenderness due to the supracondylar process. X-ray images can demonstrate the process. Ultrasonography, MRI and CT-scan are rarely helpful. EMG also can fail in demonstrating the decrease of sensory or motor conduction velocity and denervation signs. Surgical decompression is the treatment of choice through resection of both the supracondylar process and the ligament of Struthers.
